The crown (vertex) is one of the most challenging areas to restore successfully. Unlike the hairline, where hairs generally grow forward, the crown contains a complex spiral pattern that changes direction multiple times.
A successful crown restoration requires careful planning of hair direction, angulation, density distribution, and long-term donor management. Simply placing a large number of grafts into the crown does not guarantee a natural result.

Designing a Natural Crown
Every natural crown contains a central whorl or vortex from which hairs radiate in multiple directions.
Recreating this pattern accurately is essential.
Dr Young carefully designs:
- The location of the crown vortex
- Directional changes throughout the crown
- Appropriate density gradients... understand where there should be a lower density
- Natural transitions into surrounding hair

We will need to assess the vertex, and the rest of your scalp to understand the requirements. The number varies widely depending on size, current density, hair characteristics, and goals.

Transplanted hairs are generally permanent because they are typically taken from areas genetically resistant to hair loss. However, non-transplanted native hair may continue to thin over time, making long-term planning important.

Why it is important? Consultants might have a different idea of how many hairs/grafts a doctor might be able to achieve on the day, which they will quote you on and what will result in your surgery investment. in addition this will not allow continuity of care.
One of our patients recently recounted his procedure at another clinic-
" I was seen by the consultant and he drew some hairline that was quite conservative, but when I went to the clinic on the day of the procedure, he wasn't there, and the technician then drew something much more aggressive, and then made it even more lower than I wanted. " This patient has requested a full correction of his hairline that this "other" clinic created.
(It is also part of Medical Board Guidelines (1/7/2023) whereby the first cosmetic consultation must be done by the doctor performing the procedure)
On the day of your procedure, the doctor will see your donor area for the first time, meaning if the doctor foresees any issues achieving quotes results, you will only be advised of this right before the operation. In the case of some clinics there are FIFO workers who will attend to you for your booked surgery, and this is the first time you meet them.

FUE creates tiny circular extraction sites that usually heal as small dot-like scars, which are often difficult to detect once healed, particularly when performed conservatively and strategically.

No, shaving is NOT medically necessary. it is only done for procedural convenience, as it simplifies the process of extraction and implantation, especially when clinics use technicians and use the stick and place method of implantation.

- The donor is prepared by shaving the area to keep the hairs 2-3mm long.
- Hair follicles are extracted one by one from the donor area of the hair using cutting-edge instruments. The most important is the punch that rotates, oscillates, and has an adjustable depth, allowing Dr. Young to set exact extraction depth to suit the follicles and the scalp. This reduces the possibility of any injury to the healthy hairs.
- The donor area is usually the back of the head, where the hair is not affected by hair loss. Dr. Young will consider the number of hairs required and the density of follicles in the donor area against each when deciding how many follicles should be extracted and implanted. This is to prevent over-harvesting from the donor area.
- The follicles will then be prepared for implantation. They are separated into groups of hairs, sorted and placed in an active plasma solution that assures the health of the extracted hair follicle.
- The extracted hair follicles are placed into the area of concern. The implantation is done very carefully to correspond with the direction of the growth of your existing hair to restore a natural-looking hairline, so that it is undetectable if you even had a hair transplant.
- Beard to Crown is also possible.
The spiral pattern and direction changes make crown restoration very difficult. In addition, there is elevation changes of the follicles and finaly is the positioning of the patient to perform this restoration area.
This will depend on your work, we would advise a few days off work, with the knowledge that there can be no heavy lifting, dust, or heat, and that there will be forehead swelling from day 3.
